Use the form below to initiate a new home warranty service request. Please fill out and fax to 609-704-1400 Attn: Service or email to service@suprememodular.com
If you have an emergency that needs an immediate repair to mitigate further damages AND you are not able to contact your Supreme Modular representative, please have a qualified contractor make the repair. Have the contractor document the issue and be sure to hold on to your receipt for work performed. Once the work has been performed, fill out the warranty request form below, submit the form with your receipt and any photos that you have for review.
